AGREEMENT OF RELEASE AND WAIVER OF LIABILITY

It is your responsibility to inform the teacher/instructor of limitations you have before the program/class begins.
Physical Activity:
I [on behalf on my children] understand and agree that yoga includes physical movements as well as an opportunity for relaxation, stress re-education and relief of muscular tension. With any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I or my children experience any pain or discomfort, I or my children will listen to my body[‘s], discontinue the activity, and ask for support from the teacher. I [on behalf of my children] assume full responsibility for any and all damages, which may incur through participation.
Yoga is not a substitute for medical attention, examination, diagnosis or treatment. Yoga is not recommended and is not safe under certain medical conditions. By signing, I [on behalf of my children] affirm that a licensed physician has verified my good health and physical condition to participate in such a fitness program. In addition, I [on behalf of my children] will make the teacher aware of any medical conditions or physical limitations before class.
If I am pregnant, become pregnant or I am post-natal or post-surgical, my signature verifies that I have my physician's approval to participate. I also affirm that I alone am responsible to decide whether to practice yoga and participation is at my own risk.
Outdoor Programming
I [on behalf of my children] understand and agree that some on TTY's programs take place outside and that there are risks of harm associated that exist with participating in TTY outdoor programs which may give rise to bodily injury and/or property damage. These risks include, but are not limited to, (type in additional risks here) equipment failure, inadequate safety equipment, hazards associated with strenuous activity, the unavailability of adequate medical care, exposure and emergencies related to heat or cold weather, personal injury including serious physical and/or mental trauma or death, exhaustion, dehydration, broken bones, concussion, torn appendages, dislocations, bruises, cuts, infections, and any other injuries that may result in physical contact with others.
COVID-19
I [on behalf of my children] understand and agree the contagious nature of COVID-19 and voluntarily assume the risk that my children and I may be exposed to or infected by COVID-19 by attending classes and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I [on behalf on my children] understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to teachers, instructors, volunteers, and program participants and their families.
Photography
I [on behalf of my children] grant TTY the absolute and irrevocable right and permission to use, in respect to photographs, videos, motion pictures, recordings, or any other media (hereinafter collectively called (Images”) that TTY has taken of me, or my property, or my children or minors in my care, or in which I may be included with others, to copyright the same, in TTY’s own name or otherwise, to use, re-use, publish, re-publish, post, display, in printed form or any other media now and hereafter known, and for any legitimate purpose whatsoever. The hereby to TTY the right to use and publish images and photographs to further promote TTY’s business and services, including portfolio, print, website, social media, display, advertisement and editorial use.
I hereby agree to irrevocably release and waive any claims that I have now or may have hereafter against The Tripping Yogi/Theresé Julo, its owners, officers, employees, volunteers, teachers, instructors and participants. This includes releasing TTY from any liability that might arise from TTY’s or my own negligence, carelessness or from my general program participation, or traveling to/during/from TTY programs.
I have read and fully understand and agree to the above terms of this Agreement and Release of Waiver of Liability. I have had the ability to ask questions even if I did not ask any. I am signing this agreement voluntarily and recognize that my signature serves as complete and unconditional release of all liability to the greatest extent allowed by law.
